Brown or dark marks on the skin — often called age spots or liver spots — are one of the most common reasons patients come to see us at Adare Dermatology Clinics. They tend to appear on the hands, décolletage, face, and neck: the areas that get the most cumulative sun exposure over a lifetime. The good news is that most of these marks respond very well to treatment. The important caveat is that not every pigmented mark is simply sun damage, and treating the wrong one with a laser can be genuinely dangerous. That is why every pigmented lesion referred for removal at our clinics is assessed first by one of our doctors who specialise in dermatology and dermoscopy.

Why Every Pigmented Lesion Needs a Doctor's Eye First

Dermoscopy is a non-invasive technique that allows a trained specialist to examine the structures within a pigmented lesion — well beyond what is visible to the naked eye. At Adare, this assessment is never skipped, because a lesion that looks like an innocent age spot on the surface can, on dermoscopic examination, reveal features that need further investigation. Getting this right at the first visit is what keeps treatment both effective and safe.

Benign Pigmentation: Excellent Options for Removal

Once a lesion has been confirmed as benign — true photoageing pigmentation rather than anything of concern — we have several highly effective treatments available, chosen according to the lesion, its location, and your skin type:

  • IPL (Intense Pulsed Light): Broad-spectrum light energy that targets excess pigment and is well suited to larger areas of sun damage on the face, hands, and décolletage.

  • Q-switched Nd:YAG laser: A precise laser that shatters pigment within a lesion, ideal for discrete, well-defined age spots and freckling.

  • CO2 laser: A resurfacing option that can address more textured or raised pigmented lesions alongside general skin quality.

Each of these can deliver excellent, natural-looking results — but only once we are certain the lesion in front of us is what it appears to be.

Some Pigmented Lesions Are Not What They Seem

This is the crucial point behind our approach. Not every brown mark on the hands, face, or décolletage is a simple age spot. Some pigmented lesions are actually:

  • Pigmented actinic keratosis: A sun-related, pre-cancerous change.

  • Pigmented basal cell carcinoma (BCC): A skin cancer that can mimic a benign brown mark.

  • Lentigo maligna or lentigo maligna melanoma: The most serious possibility, and one that must never be missed.

It is precisely because these serious lesions can look deceptively like ordinary age spots that lasering them would be the wrong — and unsafe — approach. Any lesion of concern needs to be surgically excised and sent for histology, not treated with light-based devices. This is a non-negotiable part of how we practise, and it is the entire reason our specialist assessment comes before any discussion of laser or IPL treatment.

Multiple Pigmented Lesions? Start With a Full Body Mole Screening

If you are presenting with more than one pigmented lesion, or you are simply unsure how many marks on your skin need attention, the safest and most sensible first step is not spot treatment of individual areas — it is a full body mole screening. Prevention Is Always Better Than Cure

Whatever the underlying diagnosis — photoageing, melasma, or post-inflammatory hyperpigmentation — the single most important factor in keeping pigmentation away long-term is a consistent, correctly chosen daily skincare regime.
